Health and Wellbeing Team

  • We have 28 Health and Wellbeing Coaches (H&WC) who are employed by the Council and cover the whole county. They can offfer support to anyone aged over 16 - 100+.
  • If a customer has multiple and/or complex health or social needs, is facing crisis, and wants to make positive changes to their lives, a H&WC will be able to give them the support they need, often adopting a 'slow fix' approach.
  • H&WC adopt a coaching approach; this does require the person to take personal responsibility for changes needed, and will be willing to make those positive changes.


How do I refer to this organisation?

H&WC do not have a formal referral form. All contact should be made through a conversation. There is a duty H&WC available each day in the north, west and south of the county and therefore any initial contact should be through the Adult Social Care Single Point of Access (SPA) for the district in which the customer lives.
